Prefer sentences like “Operator A recited the phrase from sealed medium X while Operator B confirmed the checksum on device Y” over “the seed was securely transferred.” The first sentence can be checked; the second cannot.
Name refusals
Good notes state when a handoff must stop: missing witness, damaged seal, or unverified medium. Silence on refusal conditions is how rushed transfers get written as if they were normal.
Do not invent authority
